INBOUND CADET GUIDE AY22-23

Welcome to Air Force ROTC Detachment 250 at Iowa State University! Completing the Inbound Cadet Actions Worksheet is the first step towards officially joining AFROTC and pursuing a commission as an officer in the United States Air Force or Space Force! This is a three-part application process all applicants must complete to be officially enrolled in AFROTC Det 250. Failure to complete and/or provide the listed documentation will result in a delay of enrollment and/or dismissal from the program. Information on all the tasks you need to complete is broken down into three categories based on the timeline you need to complete that section by. These phases are immediate, BEFORE orientation, and AT orientation. This page can be used as your checklist. Instructions, and the required form (if applicable) are linked in the latter portions of this guide.

Immediate

Apply for a WINGS account

BEFORE Orientation

Upload the follow to your WINGS profile

  • Apply for AFAS Courses (Complete whenever you enroll for your courses)
  • Copy of ACT/SAT scores (whichever is applicable)
  • (Males only) Selective Service Number
  • Consent for Release of Student Records Memorandum
  • FM 2005 – Privacy Act Statement – Heath Care Records
  • AF Form 28 – AF Sports Physical (MUST be completed by a Physician)
  • (Scholarship Only) AF FORM 2058 – State of Legal Residency
  • (Scholarship Only) Signed Cadet Pay Documentation
  • (Scholarship Only – OPTIONAL) Service Members Group Life Insurance
  • Copy of all documentation of a civil involvement regardless of disposition if any (information regarding any involvement with civil, school, or military authorities since age 13; i.e. traffic citation; detention or suspension; article 15)
    • Event Details (e.g. speeding – 76 in a 65, fistfight, minor in possession, etc)
    • Date (Month and year)
    • Institution issuing finding (city vs state police; name of the city, school or base)
    • Result/Outcome (e.g. paid fine and amount, in-school suspension)
    • Copies of Tickets and Court Records are preferred
  • Certificates or Documentation demonstrating (as applicable)
  • JROTC Certificate or letter from JROTC Commander
  • Civil Air Patrol (CAP) Mitchell, Earhart, or Spaatz awards
  • Eagle Scout Certificate w/Gold Palm award or Girl/Senior Scout w/Gold Palm Award
  • (Prior Service Members Only) DD 214
  • (Prior Service Academy or Officer Accession Training Only) DD FORM 785
  • Marriage Certificate (if applicable)

AT Orientation

Complete and bring the following to Orientation for signing with Cadre as witnesses.

  • Social Security Card – MUST BE THE ORIGINAL
  • Birth Certificate – MUST BE THE ORIGINAL
  • AF FORM 2030 – USAF Drug & Alcohol abuse cert
  • DD FM 93 – Record of Emergency Data
  • Mail Access Authorization Release
  • FM 3010 – USAF SoU for Dependent Care Responsibility
  • Drug Demand Reduction Program MOU
  • DD FORM 2983 – Recruiter/Trainee Prohibited Activities Acknowledgement
  • (Scholarship Only) Scholarship Statement of Understanding (SoU)

INSTRUCTIONS FOR ALL DOCUMENTS

NEVER sign a document without reading AND understanding it first. All documents are legally binding documents to which you will be held liable for any and all listed terms and conditions. Make sure you understand the entire document. If you have questions, ask a competent authority to clarify the section(s) that do not make sense to you.

How to Label Application Documents

When saving AFROTC Documents, please label them in the following manner:
Last Name, First Initial – Form Number or Name
Example: Anderson, A – DD FM 93.pdf

How to Scan Documents for Uploading to WINGS

Neat, legible scans of all documents are required. Illegible documents will not be accepted and may delay your enrollment. If you do not have a scanner at home, check with your local or school libraries, or use apps designed for scanning documents on your phone. Unless you have a military CAC for digital signatures, all forms must be signed in black or blue ink. It is suggested you electronically complete all fields on the document before printing it for signature to ensure legibility.

IMMEDIATE: APPLY FOR A WINGS ACCOUNT

WINGS is an online web-portal that is initially used as your recruiting profile. Once you are certified as a Cadet, your WINGS profile becomes like a HR folder where we put your informational briefings, PT test scores, and grades, among other things. This profile will follow you throughout your cadet career, so make sure you sign up using a personal email account you will have access to for the next several years. This is the same WINGS account you sign up for after meeting with the recruiter or to apply for an AFROTC scholarship.

  • Open the Web Based Application Guide
  • Navigate to https://wings.holmcenter.com/
  • Closely follow the application guide you downloaded to complete the registration process.
  • Special notes to pay attention to
    • DO NOT use a .edu or .org email address. Only use a personal email address you will have access to for your entire college career.
    • After you provide a user name and password, WINGS will email you a confirmation code. Be sure to check your spam box.
    • Ensure you keep your username and password safe since you will need this not only as you continue your application process, but also throughout your cadet career.

BEFORE ORIENTATION (FORTE): INBOUND CADET ACTIONS

Each of the following documents will need to be completed, scanned, and uploaded to your WINGS profile BEFORE you show up for orientation.

  • Apply for AFAS Courses: Enroll in the following courses when you enroll for your other university academic coursework
  • Incoming Freshmen and current college applicants with 4 or more years of college left:
    • Apply for AFAS 103 AND AFAS 141
  • Anyone with less than 4 years of college left, contact the recruiting officer to discuss the appropriate classes to enroll in.
  • Copy of ACT/SAT scores (whichever is applicable): Scores are available online for both the ACT and the SAT via their respective websites:
  • (Males only) Selective Service Number: You will enter your number as part of your WINGS application under the Military Background tab. If you do not know your selective service number, follow the steps below:
  • Navigate to https://www.sss.gov/verify/
  • Enter your Last Name, Social Security Number, and Date of Birth
  • Check the box indicating you are not a robot
  • Submit the Form
  • Once your number is displayed, copy and paste your number into your WINGS profile under the Military Background Tab
  • Consent for Release of Student Records Memorandum: This memorandum allows us to pull your academic records for official purposes
  • Download the file here
  • Use Adobe to fill out the fillable information
  • Print the form
  • Sign in black or blue ink
  • Scan the document back in using the required naming format
  • Upload to your WINGS profile
  • FM 2005 – Privacy Act Statement – Heath Care Records: This form allows Detachment 250 and the Department of Defense to collect information from your health care records to determine eligibility to serve in the United States Military.

  • Fill out fields 6 and 7 (7 should auto populate with today’s date)
  • Print the form
  • Sign in black or blue ink
  • Scan the form and save it using the prescribed naming format
  • Upload the PDF to your WINGS profile
  • AF Form 28 – AF Sports Physical (MUST be completed by a Physician): This physical is valid for 1 year (365 days) from the date it is completed. The physical must cover the entire academic year for you to be eligible to participate in physical training. Thielen Student Medical Center is available on campus if you are unable to complete with you primary physician.
  • Download the file here
  • Type your name in Section I
  • Print the form
  • Get a sports physical conducted by a physician – they MUST use this form to be a valid physical for AFROTC
  • Once the appointment and the form is complete, scan it and save it using the prescribed naming format
  • Upload the PDF to your WINGS profile.
  • (Scholarship Only) AF FORM 2058 – State of Legal Residency: This form is only required for scholarship cadets and establishes your home of record; that is where you have residency established.

  • Scan the form and save it with the prescribed naming format
  • Upload the PDF to your WINGS profile
  • (Scholarship Only) Signed Cadet Pay Documentation: This file indicates the location you wish to have your stipend and other AFROTC related payments direct deposited to.

    • Fill out all fields – The instructions for the form are located on page 2 of the document
    • Print the form
    • Sign in black or blue ink
    • Scan the form and save it using the prescribed naming format
    • Upload the form to your WINGS profile
    • (Scholarship Only – OPTIONAL) Service Members Group Life Insurance (SGLI): SGLI is a program that provides low-cost term life insurance coverage to eligible Airmen. It is available in $50,000 increments up to a maximum of $400,000. ONLY COMPLETE THE FOLLOWING IF YOU ELECT TO SIGN UP FOR SGLI. THIS IS OPTIONAL.

    • Sign in blue or black ink
    • Scan and save the form using the designated naming format.
    • Upload to your WINGS profile

    • Save the file as LAST NAME, FIRST INITIAL – CI MONTH YEAR
    • Upload each PDF to your WINGS profile
    • Complete the Civil Involvement sections in your WINGS portal
    • As a reminder, the following information is required for each;
      • Event Details (e.g. speeding – 76 in a 65, fistfight, minor in possession, etc)
      • Date (Month and year)
      • Institution issuing finding (city vs state police; name of the city, school or base)

    AT ORIENTATION (FORTE): ITEMS TO BRING WITH YOU

    • Social Security Card – MUST BE THE ORIGINAL – You must bring with you your ORIGINAL social security card. You will receive this document back.

    • Birth CertificateMUST BE THE ORIGINAL– You must bring with you your ORIGINAL birth certificate. If you do not have an original, the only alternative is to bring a CERTIFIED copy from the courthouse of the city or county in which you were born. You will receive this document back.

    • DD FM 93 – Record of Emergency Data: This document, also known as your RED lets us know who to contact in case of an emergency and lists your beneficiaries.

    • Electronically fill out all fields EXCEPT the signature and date fields. Instructions for how to fill out the form are located on pages 3 and 4 of this document.
    • Bring the printed and completed form to Orientation. You will complete and sign this form with a member of cadre as a witness.

    • Mail Access Authorization Release: The detachment receives official mail on your behalf. This authorizes Detachment staff and cadre to access and open your official mail sent to the Detachment.

    • FM 3010 – USAF SoU for Dependent Care Responsibility: Even if you are single and have no dependents, this form must be completed and acknowledged.

    • Print the form
    • Sign in black or blue ink
    • Bring the printed and completed form to Orientation. You will complete and sign this form with a member of cadre as a witness.

    • AF FORM 2030 – USAF Drug & Alcohol Abuse Cert: This form requires you to list any and all previous illegal drug and alcohol abuse.

    • ONLY fill out the name and SSN field of the form. All other sections must be read to you by a cadre member. This will be conducted at the time of orientation.
    • Print the form
    • Bring the printed form with you to orientation. You will complete and sign this form with a member of cadre as a witness.

    • Drug Demand Reduction Program MOU: : You acknowledge you may be drug tested at any time throughout the ROTC program and failure to provide a sample for urinalysis will result in removal from the program.

    • DD FORM 2983 – Recruiter/Trainee Prohibited Activities Acknowledgement: Acknowledge certain relationships are prohibited between recruiter/cadre/staff members and trainees (cadets).
